In collaboration with UW-Extension and PATCH (Providers and Teens Communicating for Health), Youth Wellness Team members across the state of Wisconsin help support local and tribal health departments in their efforts to improve youth engagement and youth health in their communities using the Youth Advocates for Community Health model.
